EXHIBIT A <br /> SCOPE OF WORK <br /> I. Claims Management Services: <br /> A. The Service Provider shall provide claims management services associated with the City's <br /> workers' compensation program. Services to be provided include: <br /> 1. Manage all workers'compensation claims with a date of injury during the service <br /> period as long as this or subsequent Service Agreements remain in force. <br /> 2. Maintain all original claim files for the prescribed statutory period. It is <br /> understood that the files remain the City's property. <br /> 3. Receive,review and analyze all petitions for workers' compensation made by <br /> claimants. Investigation by claims administrator to determine claim validity and <br /> compensability. This investigation will include personal contract with the <br /> claimant,employer and medical providers. <br /> 4. Write initial review,action plan and outcome goals for each time loss and/or large <br /> medical claim and regularly transmit information to City of Everett Workers' <br /> Compensation Staff. <br /> 5. Include in the progress notes the following information/documentation:telephone <br /> contacts and conversations,written correspondence, investigations,diary reviews, <br /> supervisor file reviews, authority requests,authorizations,report summaries, <br /> strategy notes,examiner analyses,payment records and changes,delays,denials, <br /> and any other information that is material to the proper handling of the claims <br /> within a reasonable time from when the information is received,either orally or in <br /> writing. <br /> 6. Manage and administrate all workers' compensation claims according to the rules, <br /> guidelines and regulations outlined by the Washington State Department of Labor <br /> and Industries. <br /> 7. Establish and maintain realistic claims reserves for all anticipated benefits and <br /> related cost exposures on each claim. Review at least monthly or as the claim <br /> indicates. <br /> 8. Promptly respond to questions and requests from Workers' Compensation staff <br /> and injured workers. <br /> 9. Attend meetings with City of Everett personnel as requested. <br /> 10. Prepare written notification to the City's excess carrier when claim reserves reach <br /> 50%of the self-insured retention levels. Comply with reporting requirements to <br /> the workers' compensation excess carrier. <br /> Page 12 <br /> Workers Compensation PSA 2018 <br />
