A. Has been convicted within the last ten years of any felony that directly <br /> relates to the teaching position. By way of example only, such felonies <br /> might include: crimes against vulnerable persons, such as children, the <br /> elderly, or the disabled; crimes of dishonesty; or crimes using, or <br /> threatening, violence, including, but not limited to, the use, display or <br /> threat of a weapon. <br /> B. Has behaved in such a way that the Service Provider reasonably <br /> concludes that the proposed teacher does not possess the skill, care and <br /> judgment necessary to be a recreation teacher with due regard for the <br /> students, facility, or other persons who may be in or near the class. <br /> 25. CERTIFICATION AND DOCUMENTATION required as follows: <br /> a) ®Yes I INo Current First Aid/CPR card required for a staff on <br /> site during the instruction of these classes. <br /> b) Provide Certificate of insurance for the duration of the contract listing <br /> City of Everett as additionally insured (#11.A2) If the insurance <br /> expires prior to the end of the contract, a new Certificate of Insurance <br /> must be received by the Point of Contact by the expiration date of the <br /> previous term. <br /> c) Provide an Additional Insured Endorsement on the insurance as <br /> stated in contract (#11D). <br /> d) Provide a UBI number (#16). <br /> e) Return two signed and completed original contracts <br /> Specific Requirements for FirstSwing Camps <br /> Provide leadership and planning for baseball and softball camps <br /> a) All FirstSwing staff members will pass background checks conducted by <br /> FirstSwing. <br /> b) FirstSwing will monitor, track and be responsible for all program volunteers and <br /> interns including maintaining on file volunteer applications signed by <br /> parents/guardians of participants under the age of 18 that identify the volunteer <br /> program as a separate FirstSwing sponsored program. <br /> c) FirstSwing will provide clinics with instruction for all registered participants. <br /> d) FirstSwing will provide equipment and course syllabus. <br /> e) FirstSwing and the lead counselor will be available at all times during camp hours <br /> via cell phone provided by FirstSwing or by use of a personal cell phone. <br /> f) FirstSwing will designate a lead counselor on site during all camp hours to <br /> supervise in the absence of the Head Instructor. <br /> g) FirstSwing will monitor and evaluate student's progress. <br /> h) FirstSwing will maintain and submit course evaluations and attendance rosters <br /> provided by the Point of Conduct after each session; prior to submitting an <br /> invoice for payment. <br /> i) The FirstSwing Foundation will be responsible for paying in full all applicable <br /> scholarships as authorized by FirstSwing. Authorized is defined as signing off on <br /> the "Scholarship Tracking Form" which will be used to monitor all scholarships <br /> awarded to participants from FirstSwing or any other community scholarship <br /> program. The final invoice will not be paid until all scholarships are paid to the <br /> City. <br /> j) Students and activities will be monitored for safety. <br />
