Along grid lines B <br />and 4 anchor bolts were not installed. Also, per detail 7 on S4 and detail 4 on S3, 44 bars not epoxied into existing concrete. <br />It appears that holes were drilled and reinforcing steel installed without epoxy. Typical slab reinforcement was #4 bars placed <br />at 24" on center per detail 3 on S3. Total of (110) cubic yards of Miles Sand & Gravel mix 4026006P placed by boom pump <br />and mechanically consolidated.
