Project: WFP East Clearwell Roof Replacement <br /> Change Order No.02 <br /> Change Order Effective Date: 6/15/18 <br /> CONTRACTOR <br /> By Date: 04 <br /> �9►,n 4,,,AA5,AP0fficer <br /> Exhibit A - Description of Changed Work <br /> Work Change Directive—No. 007 Remove Existing Asphalt: WCD-007 is a lump <br /> sum revision for $3,889.46. WCD-007 added one calendar day. <br /> Justification: The scope of the project included removal and replacement of the Screen <br /> House Bridge. The existing bridge that was removed, sat at an elevation that was <br /> approximately 2-inches higher than the surrounding area. To transition from the ground <br /> to the bridge an asphalt approach had been installed. The new bridge that was installed <br /> sat an elevation that matched the surrounding area. The asphalt approach was now a trip <br /> hazard. <br /> WCD-007 covers the additional costs to break up, load, haul and dispose of the asphalt. <br /> WCD-007 also covers the additional costs to place and grade crushed rock in the area <br /> where the asphalt was removed. <br /> Work Change Directive—No. 008 Provide and Install Ladder: WCD-008 is a lump <br /> sum revision for $4,000.00. WCD-008 added one calendar day. <br /> Justification: During the design submittal review process, the hatch locations and sizes <br /> were revised. The 24" x 24" sample hatch, located in the northeast corner of the <br /> clearwell, was revised to a 36" x 36" access hatch. WCD-008 adds a ladder for the <br /> access hatch. <br /> Contract Time: <br /> Fourteen Calendar Days are being added to the contract by this change order.
