5. Repair the slab and reinforcing damage at the underside of the slab at the roof access gate <br /> location. This will involve chipping of unsound materials, cleaning and coating of existing <br /> reinforcing, potential addition of supplemental reinforcing, and concrete patching. <br /> 6. Fiber wrap at precast beams with shear cracks. This is expected to involve between two and <br /> twelve locations. <br /> 7. Fiber wrap and epoxy injection at a cracked column location, below the roof, at Grid E/21. <br /> 8. Clean and add a protective coating to steel shims at existing beam corbels and patch loose <br /> grout. This is expected to include roughly a dozen locations. <br /> 9. Add a drain near the central stair at the roof level. This is expected to be tied into the drain <br /> line for an adjacent, existing drain. A limited, sloped concrete overlay may be required near the <br /> central stair, to encourage drainage away from the stair to the new drain. <br /> 10. Replace the existing joint material in the roof-level seismic joint, near the center of the <br /> structure. <br /> 11. Patch the roof-level membrane. This may involve several dozen locations, and is expected <br /> to occur in conjunction with slab patching per Item 3 above. <br /> B. SCOPE OF SERVICES <br /> 1. Site Review: Reid Middleton will conduct a site walk with the City of Everett and members of <br /> the consultant team. The purpose of this walk is to review the issues noted in the letter report <br /> and the waterproofing consultant's recommendations, and to verify the intended strategies <br /> with the City. This will be part of, or shortly after, a project kick-off meeting. <br /> 2. Background Drawing Creation: Reid Middleton will use PDF drawings of the existing facility as <br /> a basis for the creation of CAD or Revit drawings. These drawings will form the backgrounds for <br /> the creation of a repair drawing set. The level of detail of these drawings will be sufficient for <br /> generation of repair drawings only, and is not intended to provide a full record set for the <br /> structure. <br /> 3. Repair Drawings: Reid Middleton will use the backgrounds noted above to create a repair <br /> drawing set suitable for permit submittal. The repair drawings will document intended repair <br /> locations and typical repair details. Architectural finish information will be provided by <br /> Dykeman Architecture, acting as a subconsultant to Reid Middleton. Specifications for the <br /> intended repairs are also anticipated. Specification sections for Division 00 and 01 information <br /> will be provided by the City of Everett, and is outside the scope of this proposal. <br /> 4. Cost Estimation: Reid Middleton's subconsultant, ProDims, will provide a cost estimation <br /> mid-way through the design phase and update the estimate at the permit submittal. <br />
