All food service equipment must be listed by the Natioual Sanitation Foundation (NSF) for its intended use. 2. A rzduced pressure backflow prevention device is required at the end of the copper water pipe prior to the czrbonation device. 3. Extra wall protection is required on walls behind .sinks and food preparation tables. A 16 inch high backsplash of plastic laminzte, fiberglass reinforced plastic or equal is acceptable. Plastic coated hazdboazd is not acceptable. 4. A horizontal separation of at least l6 inches or a vertical p:utition 16 inches in height is require� between the food preparation sink and all other sinks. 5. A horizontal sepazation of at least 16 inches or a vertiral partition 16 inches in height is required between the cle:n dish table, in the dishwash area, and the mop sink. 6. The ceiling above the food prepazation areas must be nonabsorbent, smouth, and easily cleanable. 7. A fee of$z43.00 was submitred with the Permit Application for Food Service Establishments. This fee is incorrect for the 65 seat restaurant indicated on the pertnit appiicaticn. The total permit fee is $251.25. An additional $38.25 must be sut�mittr,d or the restaurant seating reduced to 48 seats. Please refer ro the enclosed fee schedule for cumnt permit fees. A preopemtionel inspection is required prior to opening for business. At the time of inspection the construction of the food service establishment must be complete and all equipment must be in place. Incomplete construction may result in a $55.00 reinspection fee. Pleuse contact the Food Program office about one week in advance to schedule an appointment. This will ensure compliance with the Rules and ReQulations of the State Board of Health for Food 'iervice Sanitatien.
