All food service equipment must be listed by the Natioual Sanitation Foundation (NSF) for its <br /> intended use. <br /> 2. A rzduced pressure backflow prevention device is required at the end of the copper water pipe <br /> prior to the czrbonation device. <br /> 3. Extra wall protection is required on walls behind .sinks and food preparation tables. A 16 inch <br /> high backsplash of plastic laminzte, fiberglass reinforced plastic or equal is acceptable. Plastic <br /> coated hazdboazd is not acceptable. <br /> 4. A horizontal separation of at least l6 inches or a vertical p:utition 16 inches in height is require� <br /> between the food preparation sink and all other sinks. <br /> 5. A horizontal sepazation of at least 16 inches or a vertiral partition 16 inches in height is required <br /> between the cle:n dish table, in the dishwash area, and the mop sink. <br /> 6. The ceiling above the food prepazation areas must be nonabsorbent, smouth, and easily cleanable. <br /> 7. A fee of$z43.00 was submitred with the Permit Application for Food Service Establishments. <br /> This fee is incorrect for the 65 seat restaurant indicated on the pertnit appiicaticn. The total <br /> permit fee is $251.25. An additional $38.25 must be sut�mittr,d or the restaurant seating reduced <br /> to 48 seats. Please refer ro the enclosed fee schedule for cumnt permit fees. <br /> A preopemtionel inspection is required prior to opening for business. At the time of inspection the <br /> construction of the food service establishment must be complete and all equipment must be in place. <br /> Incomplete construction may result in a $55.00 reinspection fee. Pleuse contact the Food Program <br /> office about one week in advance to schedule an appointment. This will ensure compliance with the <br /> Rules and ReQulations of the State Board of Health for Food 'iervice Sanitatien. <br /> If there are any significant changes or additions to your layour. or equipment, the Snohomish Health <br /> District must be notified. <br /> Please contact me if you have any questions.
