Plumbing permit application. <br /> 2. Letter of appointment as a General Contractor. <br /> 3. Full plumbing plans. <br /> 4. Copy of the submitted Health Questionnaire to the Snohomish Health Department. <br /> 5. Copy of the Health District letter. <br /> 6.
