Dear Ms. Hagedorn, Thank you very much for taking the time to review the health questionnaire I submitted in support of"Eight Ball Café"bar/restaurant opening in downtown Everett. I also thank you for taking the time to visit the location and provide much needed guidance,which helped me as a new restaurant business owner to navigate In reference to your letter dated October 12,2017,following are the responses: 1. An additional preparation sink will be installed to ensure preparation sinks dedicated to meats and fruits/vegetables separately, and in accordance to health requirements. A proper separation will be installed between preparation and hand sinks to avoid any cross-contamination. 2. Self-service buffet will no longer be part of this concept,no such equipment will be available in the premises. 3. An NSF compliant three-compartment sink is available on site,with drain boards on either side. The architectural drawings will reflect draining boards as required. 4. All equipment on site are or will be NSF and/or UL listed. A complete list of equipment is provided for your review. 5. Hand sink in the piz7a preparation area is relocated per your guidance,updated architectural plan reflects the relocation. 6. Panini machine will be located next to the service window between the kitchen and the piz7a preparation area,also referred to as service counter. 7. Mop sink will be located inside the broom closet by the exit to the common area. There will be what is referred to as"waitress station"in the premises,just a cream/sugar and dirty dish return in a self-service fashion. 8. Submitted revised plan is resealed as requested to ',4"equals 1 foot as requested. 9. An additional hand sink was placed in the east side wall of the kitchen,in addition to the one already placed at the entrance of the kitchen,to ensure that the 20' radius rule is met and hand sink accessibility near kitchen door and dishwasher. 10. The walk-in cooler's dimensions are 8' wide, 10' long and 10' tall. 11. As indicated in item#4 above,a complete equipment list is submitted for your review. It shows an NSF and/or UL listed and approved equipment list.
