1 Section 4: If at the time the field serviceman calls at a customer's residence 2 and contact is not made with the customer, he will leave a notice stating substantially 3 as follows: "Your unpaid account amounts to $ . If you question whether 4 the amount shown is due, you must immediately contact (title of authorized 5 representative to review disputed bills and correct errors) at between the 6 hours of and , Monday through Friday and discuss the matter or make an 7 appointment with (employee or title of officer or title of person authorized to review 8 disputed bills and correct errors). Otherwise, if the amount is not paid by 9 , the service will be cut off without further notice." 10 Service will be restored during regular hours only after the full amount due and a 11 $15 reconnection charge has been paid. 12 Section 5: When the water has been shut off, a notice shall be left notifying 13 customer. 14 Section 6: The Budget and Finance Director, or person he designates, shall be 15 authorized to review disputed bills and correct errors. Customers who question or 16 dispute their bills, shall have the right to an informal conference with an employee or 17 officer of the City so authorized by the Budget and Finance Director to review disputed 18 bills and correct errors, during regular business hours, 8:00 a.m. to 4:00 p.m., Monday 19 through Friday at 20 Section 7: The Budget and Finance Department shall establish uniform 21 procedures for deferred payment arrangements. 22 Section 8: Right of Appeal. 23 A. Customers who disagree with the decisions of City officers or employees 24 after informal conferences shall have the further right to appeal to the Budget and 25 Finance Director, or such supervisory officer as the Budget and Finance Director may 26 designate. 27 B. Customers who are appealing the decision of the informal conference will 28 not be subject to water shut off during the appeal proceedings provided that undisputed 29 amounts are paid. 30 31 32 Page -2-
