Coordinated Community Response Team Participation <br /> Agency Submitting this Form City of Everett Police Department <br /> County Snohomish County <br /> Date of Community Planning Meeting 9/13/2018 <br /> Only one completed form to be submitted for your CCR team. Decide which agency will respond on behalf of all <br /> STOP Grant applicants in your county.The designated agency must submit with their application this completed <br /> CCR form. A copy of the submitted form should be provided to the other STOP Grant applicants in your county <br /> for their files. <br /> A condition of receiving these STOP Formula Grant funds is mandatory attendance at the CCR meetings and <br /> meaningful participation as a member of the county CCR team,which must meet at least quarterly during the <br /> grant period. <br /> While it is a federal requirement that recipients report on their progress in reducing, or reducing the risk of, <br /> domestic violence related homicides, CCR teams should also consider and address services for youth or adult <br /> victims of domestic violence, sexual assault, dating violence, or stalking. During the grant period, CCR teams will <br /> be required to report on their activities.
