Decide which agency will respond on behalf of all STOP Grant applicants in your county.The designated agency must submit with their application this completed CCR form. A copy of the submitted form should be provided to the other STOP Grant applicants in your county for their files. A condition of receiving these STOP Formula Grant funds is mandatory attendance at the CCR meetings and meaningful participation as a member of the county CCR team,which must meet at least quarterly during the grant period. While it is a federal requirement that recipients report on their progress in reducing, or reducing the risk of, domestic violence related homicides, CCR teams should also consider and address services for youth or adult victims of domestic violence, sexual assault, dating violence, or stalking. During the grant period, CCR teams will be required to report on their activities. Please keep notes and sign-in sheets from your CCR meetings.
