Activities/Project Description <br /> The Subrecipient will be responsible for administering the WayOUT Program program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The WayOUT Program will serve Insert number of people to serve 78 families persons by providing <br /> Enter the program and/or activity descri at least eight (8) WayOUT seminars consisting of interactive sessions <br /> and relationship-building activities for low-risk juvenile offenders/at-risk youth and their parents or <br /> caretakers with the goal of reducing youth risk-taking behaviors, increasing supportive relationships, <br /> and preventing youth homelessness and recidivism in the legal system. Seminar instructors provide <br /> interactive, experiential presentations (teen-specific, adult-specific, and shared experiences) <br /> intended to: Support parents to increase their understanding of adolescent development, parenting <br /> styles, anger management, inherent personality differences, sources of negative behaviors, and <br /> coaching for decision-making • Support youth to increase their ability and motivation for positive <br /> decision-making, increase engagement in pro-social behaviors, and decrease engagement in risk- <br /> taking behaviors • Support families in increasing life skills around communication, problem-solving, <br /> and conflict resolution • Increase empathy and connection between parents and their teens. <br /> Program Outcomes: <br /> • 75% of parents self-rate as increased or adequate communication skills <br /> • 75% of parents self-rate as increased in adequate understanding of adolescent development <br /> • 60% of parents self-rate as improved or adequate parent-teen relationship <br /> • 60% of teens self-rate as increased or adequate communication skills <br /> • 60% of teens self-rate as increased or adequate belief in their ability to make positive choices <br /> • 60% of teens self-rate as increased or adequate teen/parent relationship . <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> Cocoon House leadership and administrative staff will provide support to ensure the WayOUT <br /> program: is operated in a manner consistent with contract requirements; maintains fidelity to program <br /> models; maintains appropriate staffing, supervision, data, and evaluation; and develops and <br /> maintains effective working relationships within the community. <br /> A-1 <br />
