Associate Director of Programs: Services, Nico Quijano — Oversees the Prevention department, <br /> including WayOUT, directly supervises the Prevention Manager and monitors program performance. <br /> This position is partially supported by this funding at .05 FTE. <br /> Prevention Manager, Faith Simonelli — Responsible for ensuring fidelity to the program model while <br /> providing a supportive and inclusive environment and supervising program staff and partners. This <br /> position is partially supported by this funding at .10 FTE. <br /> Additional Staff, Community Educators, and Partners: Other program staff and community partners <br /> provide instruction modules for the WayOUT program and include mental health providers, law <br /> enforcement, and other stakeholders. <br /> II. Time of Performance <br /> Services of the Subrecipient shall start on the 1st day of July, 2018 and end on the 30th day of June, <br /> 2018. The term of this Agreement and the provisions herein shall be extended to cover any <br /> additional time period during which the Subrecipient remains in control of CDBG funds or other <br /> assets, including program income. The City may, at its discretion, extend the term of this Agreement <br /> to allow for the expenditure of unexpended funds. <br /> III.
