All work shall be performed in accordance with the permit and current City of Everett Design and Construction Standards and Specifications. 3. Call Location Underground Service 48 hrs before you dig. TOLL FREE NUMBER 811 F_ 4.Applicant shall notify the City Public Works contact (Heather Griffin, with Mark Sadler as backup) of their intent to perform the project 3 working days in advance of any work. Applicant must also notify of any changes to their work schedule to the Public Works contact. 5. Applicant must provide their project manager contact and a 24-hour contact number for the work. 6. Standard utility notification and locate process shall be done and followed. 7. Work must follow work plan as approved by Ecology dated May 17, 2010. 8. All spoils, pipelines, structures and water encountered during the work must be properly stored on private property or properly disposed of No storage at any time is allowed within the ROW. Any water discharged to the City storm or sewer systems must be permitted with a Discharge Al ,thorization from the City's Industrial Pretreatment Work Group. 9. Applicant is required to pre -notify Kimberly Clark and arrange traffic and parking control on Federal Ave during their work. Work must be done to coordinate KC work and access needs. 10. Applicant shall provide access to their work for the city inspector or consultant at all times. 11. If the Anomaly described per page 11 of work plan is to be left in place, Applicant shall inform city inspector and/or consultant and make available for inspection. If Anomaly is left in place then as best as -built information and mapping shall completed and submitted to city. Applicant shall remove the Anomaly if at all possible. 12. Backfill shall be done immediately; pavement restoration shall be done within 3 days of completion.
