I <br /> I7. Certification of Wage Compliance: Within the three-year period immediately preceding the date <br /> of bid solicitation, not have been determined by a final and binding citation and notice of <br /> I assessment issues be the department of labor and industries or through a civil judgment entered by <br /> a court or limited or general jurisdiction to have willfully violated, as defined in RCW 49.48.082, <br /> any provision of chapter 49.46,49,48,or 49.52 RCW. <br /> I B. Failure to complete and fully execute the aforementioned forms and to submit them with the Bid may <br /> result in rejection of Bid. <br /> I C. The following forms are to be executed after the award of Contract: <br /> 1. Contract: This Contract to be executed by the successful Bidder in duplicate within 14 calendar <br /> days after the award date. <br /> I 2. Payment and Performance Bond: Both forms are to be executed by the successful Bidder and <br /> his surety company in duplicate. The amount of each bond shall be 100 percent of the Total Bid <br /> Amount and shall be submitted with the Contract. <br /> I 3. Proof of Insurance: Insurance certificates and endorsements shall be obtained and maintained in <br /> force in accordance with Document 00 72 13, Part 15 "Liability and Insurance", of the Contract <br /> Documents. <br /> I <br /> 4. Power of Attorney: Attorneys-in-fact who sign bid bonds or contract bonds must file with each <br /> bond a certified and effectively dated copy of their Power of Attorney. <br /> 5. Statement of Intent to Pay Prevailing Wage (L&I Form F700-029-000) and Affidavit of <br /> in <br /> Wages Paid (F700-007-000) from the Contractor, Subcontractor and agents to the Subcontractor <br /> shall be submitted to the Employment Standards Division, State Department of Labor and <br /> Industries, Olympia, Washington. If the prime contract is over $1,000,000, then Contractor also <br /> I needs to submit L&I Form F700-164-000 (Affidavit of Wages Paid EHB 2805 <br /> Addendum). <br /> 6. Weekly Statement with Respect to Payment of Wages (U.S. Dept of Labor Form WH347): <br /> I Contractors, Subcontractors, and agents to Subcontractors using Payroll Form WH347) may use <br /> State of Compliance found on back of form. <br /> 7. Weekly Statement of Compliance (U.S. Dept of Labor Form WH 348). Contractors, <br /> I Subcontractors, or agents to Subcontractors not using Payroll Form WH347 shall attach the <br /> Statement of Compliance Form WH 348 to each payroll. <br /> 8. Approval of Subcontractors: Contractors shall request approval of Subcontractors on a form <br /> Iprovided by the City prior to their working on the Site. <br /> 9. Construction Progress Schedule: To be submitted as required by Document 00 72 13, Part 5 <br /> "Progress and Completion", of the Contract Documents by the Contractor in accordance with <br /> ISection 01 32 16. <br /> 10. Traffic Control Plan: Standard Traffic Control Plans are included in the City's Design and <br /> Construction Standards. If, and to the extent, such plans are not sufficient, complete or adequate <br /> to support the Bidder's planned means and methods of performing the Work, the Bidder must <br /> develop an adequate, complete and sufficient traffic control plan at its cost,that shall be submitted <br /> for the City's approval prior to construction. Deviation from the Standard Plans must be <br /> I submitted by the Contractor and approved by the City prior to construction. <br /> 11. Disposal Sites: Provide the City with the location of all disposal sites to be used, and also provide <br /> copies of the permits and approvals for such disposal sites.
