Proposal Signature Sheet: To be filled in and signed by the Bidder. <br /> I3. Subcontractor List: To be filled in by the Bidder. <br /> 4. Local Agency Certification of Federal-Aid Contracts: To be certified by signing the bid <br /> proposal <br /> I <br /> 5. Non-Collusion Declaration: To be submitted with the bid. <br /> I 6. Bid Bond: This form is to be executed by the Bidder and the surety company unless bid is <br /> accompanied by a certified check. The amount of this bond shall be not less than five percent <br /> (5%) of the total amount bid and may be shown in dollars or on a percentage basis. Cash will <br /> I not be accepted. <br /> 7. Proposal for incorporating Recycled Materials into the Project: To be filled out and <br /> Submitted with the Bid. <br /> I Failure to complete the aforementioned forms and to submit said forms with the bid may be due <br /> cause for rejection of bid. <br /> I3.0 Pre-Award Forms <br /> The following form is required to be signed and submitted prior to award of Contract: <br /> I1. Certification of Compliance with Wage Payment Statutes: To be filled in and signed. <br /> This certification is not required to be submitted with the bid proposal and may be submitted <br /> Iwithin 24 hours after bid opening. The Contract cannot be awarded without this certification. <br /> 4.0 Contract Forms <br /> IThe following forms are to be executed after the award of Contract: <br /> 1 1.
