EXHIBIT A <br /> SCOPE OF WORK <br /> Specific Requirements-Nurse Wellness Services <br /> Service Provider must be a registered nurse currently licensed to practice nursing in the State of Washington. <br /> Service Provider may not provide Work unless Service Provider's license is current and active. <br /> 1. The purpose of this Agreement is to assist and educate seniors in good health practices.To this end Service <br /> provider may provide the following services: <br /> -Assistance with case management doctor appointments,and referrals to physicians and/or Emergency <br /> Room as needed <br /> -Vitals checks(e-g.blood pressure)and education; <br /> -Basic first aid;and <br /> -Health assessments(including weight checks and dietary recommendations),related counseling and <br /> coordination of health programs and promotions at the Carl Gipson Senior Center as needed. <br /> 2. Hours.The Service Provider may work 3 hours per day two days per week and as necessary to substitute <br /> for the other nurse wellness provider. The work week will consist of Monday through Friday,9:30 AM <br /> -12:30 PM. <br /> 3. Service Provider is responsible for the professional quality,technical accuracy and the coordination and <br /> provision of all Work furnished under this agreement. <br /> 4. Under no circumstance whatsoever may Service Provider provide services under this Agreement that are <br /> outside the usual and ordinary practice of nursing. <br /> 5.
