C. Specific Requirements for Helen Dilday <br /> 1. Provide the following for all Health and Wellness classes taught: <br /> a) This Professional Services Agreement is effective when the provider receives <br /> a written "Notice To Proceed" with a specific date from the provider's Point of <br /> Contact. <br /> b) Develop class: planning dates, times and best activities and topics <br /> c) Maintain attendance records. <br /> d) Assist with promotion of classes. <br /> e) Monitor and evaluate students' progress. <br /> f) Maintain and submit course evaluations. <br /> g) Monitors students and activity for safety. <br /> h) Prepare class materials and handouts for each class <br /> i) Renew Annual City of Everett Business License for 2019, no later than <br /> January 31, 2019 <br /> 2.
