EXHIBIT A <br /> SCOPE OF WORK <br /> Specific Requirements-Social Services <br /> Work to be performed: <br /> 1. Social Work and Case Management to aid seniors in assistance regarding chore services,financial aid,housing <br /> and home repair,food resources,elder abuse,Medicare,grief counseling,depression,mental health issues, <br /> support groups,respite care,transportation,etc.The social worker will work in conjunction with agencies such as <br /> Senior Information and Assistance,DSHS,Catholic Community Services, Snohomish County Case Management <br /> and Long Term Care and Aging Center. <br /> 2. Scheduled hours for Social Worker shall be 9a.m.to 1p.m either on Monday or Thursday,4 hours per session, 8 <br /> hours per week maximum not to exceed contract amount. <br /> 3. Service Provider is responsible for the professional quality,technical accuracy and the coordination and provision <br /> of all Work furnished under this agreement. <br /> 4.Under no circumstance whatsoever may Service Provider provide services under this Agreement that is outside <br /> the usual and ordinary practice of social work. <br /> 5.
