Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Safe Streets Employment program in a <br /> manner satisfactory to the City and consistent with any standards required as a condition of <br /> providing these funds. Such program will include the following activities eligible under the Community <br /> Development Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Safe Streets Employment will serve Insert number of people to serve 25 persons by providing <br /> Enter the program and/or activity descr supported employment services to unemployed adults encountering <br /> obstacles to employment related to mental health concerns, who are not engaged in, and/or are <br /> not eligible for, community mental health services. Program elements include vocational <br /> assessments, job search assistance, benefits counseling, resume writing, mock interviews, <br /> education on self-care and work attire, interpersonal relationships, discerning possible on-the-job <br /> accommodations, and facilitating connections to other community services and natural supports. <br /> The Employment Case Manager will provide these direct client services. We anticipate providing <br /> 280 Client Contact Hours during the grant period. . <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> Oversight of the Employment Case Manager is provided by the Director of Clinical Support Services, a <br /> Licensed Mental Health Counselor with twenty-two years' experience providing mental health <br /> services, with the last three years in manager/director capacity. Additional support and guidance and <br /> assistance with compliance and reporting may be provided by the Manager of Clinical Quality <br /> Improvement. <br /> The Data Integrity Specialist can provide support with our Raintree Electronic Medical Records <br /> system. The Director of Finance and Administration will provide additional oversight to ensure <br /> compliance with regulations and reporting requirements. <br /> The Director of Development and Marketing will proactively engage with local employers to build <br /> relationships and foster support of potential job applicants from the program. <br /> A-1 <br />
