Employee Benefits <br /> The City's fringe benefits contribute greatly to your annual compensation and to your personal well being.The <br /> purpose of this report is to help you better understand the value of your benefit program.This report is only a <br /> summary of your City provided benefits and every effort has been taken to ensure the accuracy of this <br /> statement. <br /> Health insurance <br /> The City provides health insurance to all full-time regutarempicyees and elected officials.You have the option <br /> of selecting from three different plans including a PPO,IMO and Kaiser Permanente aging you to select the <br /> plan that beat meats the needs of you and your family.The employee paid portion of the premium may be <br /> subject to IRS Sedan 125 pre-taxbenefrt <br /> Dental Insurance - <br /> Dental care benefits ate paidlAhrough MetLife Dental.Benefits include preventive seNces,basic care and <br /> orthodontic treatment. <br /> Vision Insurance <br /> The City contracts with Vision Service Plan(VSP)for vision care.The City currently payS fat all full-ihne <br /> employees and their enrolled dependents.Benefits include eye exams as welt as frames and lenses. <br /> Lona Term Disability Insurance <br /> The City pays the premium for all full-time employe%for this coverage.In the event you are unable to work due <br /> to an off-the-job injury or illness,you may be eligible for long fern disability benefits.Disability benefits may be <br /> available after a 30 day elimination period. <br /> Life insurance <br /> In the event of your death,your beneficiary wilt receive 550,000 or 1 times yourannualsalary,whichever is <br /> greater.The City pays the premium for alt full-lime employees for this coverage. <br /> Kentucky Public Employee Retirement System(PERS) <br /> Retirement bene is are provided under the provisions of PERS paid for by the City.The current benefit formula <br /> is2%Ei 55. <br /> Public Agency Retirement System(PARS) <br /> Enhanced retirurnent benefits are provided under the provisions of PARS paid fat by the City.The Supplemental <br /> brmulais.7%a55. <br /> 401(all Deferred Compensation <br /> The City contributes an amount equal to a percentage of your salary based en your classification.The plan is <br /> administered through ICW,Retirement Corporation. <br /> 457 Deferred Compensation <br /> A voluntary tax-deterred program is available through IGfdA and Nehru-wide. <br /> Employee Assistance Program <br /> Counseling services ate provided through ESrSCQ. <br /> CaIPERS Long Term Care Program <br /> Avolunfary self-funded program to assist with tong-lorm care expenses inducts-1g in-home ravio3s,assisted <br /> living facilities or in a nursing home.The City pays up to$+90 pet month for employees enrolled in the program. <br /> PSA between Cayenta and City of Everett 19 08/08/2018 <br />
