United States Department of the Interior <br /> NATIONAL PARK SERVICE <br /> Lake Chelan National Recreation Area <br /> < 1, ` AN. o Ross Lake National Recreation Area <br /> North Cascades National Park <br /> 810 State Route 20 <br /> 'was,wj9 Sedro-Woolley,Washington 98284-9394 <br /> IN REPLY REFER TO: <br /> Dear applicant: <br /> Thank you for your interest in applying for a Commercial Use Authorization(CUA)with North Cascades National Park. <br /> Services and activities permitted under a CUA must be appropriate for visitors to North Cascades National Park. It's important <br /> that your application include as much detail as possible about the activities you wish to receive a permit for,and that all <br /> required documents and certifications be completed and returned to the park in a timely manner. <br /> There are two separate application and/or renewal packages for CUAs: <br /> 1. One package is for first-time applicants,or Permittees who's CUA has expired and who are seeking to re-apply for a <br /> new permit; <br /> 2. The second package is for Permittees who have a current,two-year CUA,but need to submit annual information <br /> and/or updated certifications in order to keep their permit in force for the second year. <br /> This application package is for first-time applicants and those entities whose two-year permit expired and who are <br /> seeking to re-apply for a new permit. This form should be used for all commercial entities and non-profit organizations who <br /> are providing guided services in the park. <br /> There are two parts to this application package: Part 1: Application Form provides basic information that must be submitted to <br /> the park to be considered for a permit. Part 2: Supplemental Information includes additional information regarding Commercial <br /> Use Authorization Requirements and required documents that Park will need prior to approving a CUA. <br /> Please complete Part 1 of the application,and review Part 2:Supplemental Information for additional requirements regarding <br /> CUAs. Return the completed application form,along with payment of$100 for the required non-refundable application fee,to <br /> the address listed at the top of this letter. Fees may be paid on-line through pay.gov. The link is:www.pay.gov.com. Search <br /> for North Cascades National Park and choose the form"North Cascades NP Commercial Use Authorization Fees. Follow the <br /> instructions on the screen to make payment. Payment may also be made by check or money order made to"NPS-DOI". <br /> Once we receive your completed application package,we will review the form for completeness and contact you if we have any <br /> questions or need clarification. If all information is complete and meets requirements,we will provide you with a copy of the <br /> permit for your review. If you agree to the conditions and wish to continue the permit process,sign the permit and return it to to <br /> us with the annual fee totaling$150. This fee includes$100 for administration and$50 for monitoring. If payment is made <br /> electroncially,please forward us a copy of the reciept you received through pay.gov.The CUA is not effective until all of the <br /> required documentation has been received by the Park and the Permit has been signed by the Superintendent.
