activity completion within thirty (30) days of completion of each individual <br /> HOME-assisted home rehabilitation loan activity. <br /> 2. Other Reporting Requirements <br /> a. The Agency agrees to submit to the County monthly the HOME <br /> Program Income Monthly Report (Exhibit H) or similar form with the <br /> same information. <br /> b. The Agency agrees to submit to the County, thirty (30) days after the <br /> end of each quarter, the HOME Owner-Occupied Housing <br /> Rehabilitation Loan Programs Quarterly Report (Exhibit I). <br /> c. The Agency shall submit an annual audit which includes this Project, <br /> certified by the State Auditor, to the County within twelve (12) months <br /> after the end of its fiscal year. <br /> d. If requested by the County, the Agency will submit to the County prior <br /> to the disbursement of funds certified copies of: <br /> i. Its Equal Opportunity and Fair Housing marketing plan; <br /> ii. Records demonstrating that the units meet or will meet all <br /> applicable property standards; <br /> iii. Certification that no layering of federal funds has occurred or will <br /> occur; <br /> iv. A conflict of interest statement; <br /> v. Evidence of flood insurance, if applicable; and <br /> vi. Evidence that neither the Agency nor any contractor, subcontractor, <br /> agent, representative, or consultant working on the Project is <br /> debarred or suspended from participation in federal programs. <br /> e. Each such report shall be subject to the approval of the County. <br /> 3. The County may request any other information that it deems necessary to <br /> monitor compliance with the requirements set forth in this Agreement. <br /> Such information shall be provided promptly by the Agency. <br /> 4. At any time during the term of this Agreement, the County or its designee <br /> may inspect all accounting and other records pertaining to the HOME <br /> assistance activities and operation of the Project. Upon request by the <br /> County, the Agency shall notify homeowners of upcoming inspections of <br /> Exhibit A-3 <br /> HCS-16-42-1601-198(3) <br /> City of Everett <br /> Page 20 of 23 <br />
