Sent via e-mail <br /> PUBLIC 11/DRK5 et( L L( I io <br /> April 22, 2016 <br /> UNITED NORTH AMERICAN INVES <br /> 1600 148TH ST SE <br /> MILL CREEK, WA 98012-8210 <br /> Complaint: LACKS ACCESSIBLE PARKING SIGNAGE <br /> Job Address: 3809 BROADWAY <br /> Complaint No: W1603-009 <br /> RE: COMPLAINT <br /> Dear UNITED NORTH AMERICAN INVES, <br /> A complaint was received on March 25th, 2016 about the lack of accessible parking to service <br /> the building at the address listed above. <br /> Permit History <br /> Review of permit B0405-018 drawings indicates that the two accessible parking spaces shown <br /> on the attached site plan met code requirements at the time the permit was issued. <br /> Permit Application Required <br /> A building permit application is required to indicate the accessible parking signage on the site <br /> plan. To discuss minimum requirements, please contact me at mhaleyPeverettwa.gov or 425- <br /> 257-8813. A new permit application and two sets of drawings should be submitted in a timely <br /> manner such that the permit can be issued within 90 days of the date of this letter, no later <br /> than July 22, 2016. Final inspections must occur within 180 days of the issuance of the permit. <br /> All required submittals must be of sufficient detail and information to ensure that the proposed <br /> structure will be restored to its original permit configurations, or updated to be in compliance <br /> with current adopted codes. Please note that federal ADA, FHA, and Fair Housing Accessibility <br /> Guidelines and regulations may be more restrictive. Compliance with these regulations is the <br /> responsibility of the owner of the building. <br /> These comments address building issues only.
