Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Meals on Wheels program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Meals on Wheels will serve 20 persons by providing 2000 meals to homebound individuals who <br /> meet all of the eligibility criteria for Meals on Wheels except for age. Meals on Wheels recipients <br /> receive weekly deliveries of 7-14 meals which they select from a menu of 25 meals. They also <br /> receive nutrition assessments, counseling, <br /> and nutrition education tailored to their individual needs.. <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> Homage Senior Services general administrative functions include Executive, Finance, Human <br /> Resources, and Information Technology. <br /> A-1 <br />
