As the vehicle came to a curve in
the road, the vehicle failed to turn and continued straight. Veh 1 passed, through the oncoming lane
of travel, and went off road into the ditch. The vehicle struck a yellowcte pole. After it collided
with the pole, the vehicle flipped over and collided With a wooden-PUD utility pole. When the vehicle
struck the utility pole, it broke off at the base and leaned up against Veh 1.
The driver of Veh 1 stated that he was on his way home from work. He fell asleep without
remembering feeling drowsy, and woke up when his vehicle collided with something and went
airborne. He was able to get out of the vehicle and get clear of the power lines. He stated that he
didn't feel sick, hadn't taken ANY medications, and had gotten 6 hours of sleep the night before. I
administered SFST's on the driver, but didn't get any indications of intoxicants. The driver didn't have
a valid driver's license and no insurance on his vehicle.
Based on my investigation, I determined that Veh 1 was the proximate cause of the collision. The
driver of Veh 1 fell asleep at the wheel and drove off the road. I issued him tickets for Negligent
Driving 2nd Degree, No Valid Operator License with Valid Identification, and Operate Motor Vehicle
without Insurance.
I CERTIFY(DECLARE)UNDER PENALTY OF PERJURY UNDER THE LAWS OF THE STATE OF WASHINGTON THAT THE FOREGOING IS TRUE AND CORRECT(ROW 9A.72.085)
MARK GIST 11-16-18 05:36 PM
INVESTIGATING OFFICER'S SIGNATURE UNIT OR 01ST DET DATED PLACE SIGNED
APPROVED BY DATE
CRAIG DAVIS 1109 11/17/2018 7:54:37 AM
BADGE OR ID.# 1419 CR14 WA0310300 TINE POIJCE DISPATCHED 3:19 PM TIME POLICE ARRIVED 3:25 PM
PART B 3000445-160 R 17/061 PAGE I 2 I OF I 4 I
