Marine View Drive, Everett <br />Dear Ms. Symms: <br />Your plans have been reviewed with the Rules and Regulations of the State Board of Health, <br />and with the policies of the Snohomish Health District. With the addition of the ollowing, the <br />plans are approved. <br />1. The Health District operating permit application process must be completed prior to opening <br />for business. <br />2. An additional handwash sink is required at the ice cream and espresso area. <br />3. An additional handwash sink is required on the cook line. This sink must be centrally <br />located on the cook line. <br />4. An additional handwash sink is required at the scullery area. <br />5. A horizontal separation of at least 16 inches or a vertical partition 16 inches in height is <br />required between the food preparation sink and all other sinks. <br />6. All food service equipment must be listed by the National Sanitation Foundation (NSF) for <br />its intended use. The Kairak refrigerated food prep table was not found in the current NSF <br />listings. This unit must be replaced with an NSF listed unit. <br />7. Storage refrigerators must be listed for the storage of potentially hazardous foods. <br />8. Extra wall protection is required on walls behind sinks and food preparation tables. A 16 <br />inch high backsplash of plastic laminate, fiberglass reinforced plastic or equal is acceptable. <br />Floor to wall protection is required behind dishwashers. Plastic coated hardboard is not <br />acceptable. <br />9. The ceiling above the food preparation areas must be nonabsorbent, smooth, and easily <br />cleanable. <br />10. All light fixtures in food preparation and storage areas must be provided with covers or <br />shatter proof bulbs. <br />11. Plumbing must meet state and local codes. <br />12. The ventilation system shall be installed and operated to meet applicable building, <br />mechanical, and fire codes. <br />A preoperational inspection is required prior to opening for business. At the time of inspection <br />the construction of the food service establishment must be complete and all equipment must be <br />in place. Incomplete construction may result in a $60.00 reinspection fee. Please contact the <br />Food Program office at least one week in advance to schedule an appointment.
