Marine View Drive, Everett <br />Dear Ms. Symms: <br />Your revised plans have been reviewed with the Rules and Reglations of the State Board of Health, and with <br />the policies of the Snohomish Health District. it tea itto t e o owing, the plans are approved. <br />I. The Health District operating permit application process must be completed prior to opening for <br />business. <br />2. A horizontal separation of at least 16 inches or a vertical partition 16 inches in height is required between <br />the food preparation sink and all other sinks. <br />3. Storage refrigerators must be listed for the storage of potentially hazardous foods. <br />4. Extra wall protection is required on walls behind sinks and food preparation tables. A 16 inch high <br />backsplash of plastic laminate, fiberglass reinforced plastic or equal is acceptable. Floor to wall protection <br />is required behind dishwashers. Plastic coated hardboard is not acceptable. <br />5. The ceiling above the food preparation areas must be nonabsorbent, smooth, and easily cleanable. <br />6. All light fixtures in food preparation and storage areas must be provided with covers or shatter proof <br />bulbs. <br />7. Plumbing must meet state and local codes <br />8. The ventilation system shall be irstalled and operated to meet applicable building, mechanical, and fire <br />codes. <br />9. Please note that no food service will be allowed at the service stand, Item #51 on Sheet #1 (formerly the <br />ice cream and espresso area) unless a handwash sink is installed. This includes all hot food, cold food and <br />beverage service. If the service stand is customer self-service then a handwash sink is not required. <br />A preoperational inspection is required prior to opening for business. At the time of inspection the <br />construction of the fold service establishment must be complete and all equipment must be in place. <br />Incomplete construction may result in a $60.00 reinspection fee. Please contact the Food Program office at <br />least one week in advance to schedule an appointment. This will ensure compliance with the Rules and <br />Regulations of the State Board of Health for Food Service Sanitation. <br />If there are any significant changes or additions to your layout or equipment, the Snohomish Health District <br />must be notified. <br />Please contact me if you have any questions.
