CITY OF EVERETT SPECIAL PROVISIONS DIVISION P-PROPOSAL CITY OF EVERETT,WASHINGTON 1 PROJECT TITLE WORK ORDER#UP3398 , I To the City Council Everett, Washington The undersigned Bidder declares that he has carefully examined the Plans and Specifications, Notice to Contractors, Standard Specifications, Special Provisions, Appendix, Proposal, and Contract for the construction of a new pipe conveyance system to separate, collect, and treat stormwater from existing combined sewer system; rehabilitate and replace existing combined I sewer collection systems; and street/sidewalk restoration. Project involves construction of approximately 6,000 LF of new storm drainage pipe (6-inch to 24-inch dia.) with approximately 61 catch basin and manhole structures and approximately 10 water quality treatment structures. I Construction of approximately 5,300 LF of existing combined sewer replacements (8-inch to 12- inch dia.)with approximately 32 manhole structures(48-inch to 72-inch)and reconnection of 108 existing side sewers; asphalt street restoration; curb, gutter, sidewalk, and curb ramp reconstruction; and other such appurtenances and other such Work as may be necessary, in I accordance with the Specifications, as shown on the Plans. The undersigned declares that he has made such investigations as are necessary to determine the conditions to be encountered, and that, if this Proposal is accepted, he will enter into a contract with the City of Everett, Washington, in the form of Contract hereto annexed, he will, to the extent required, provide the necessary I equipment, tools, apparatus, and other means of construction, and he will furnish all labor and materials necessary to complete the Work in the manner herein specified and according to the requirements of the Engineer. 1 The undersigned Bidder certifies that this Proposal is in all respects fair and is made without collusion on the part of any person, firm or corporation mentioned below, and no officer or employee of the City of Everett is personally or financially interested,directly or indirectly, in the I Proposal or in any purchase of or sale of any materials or supplies for the Work to which it relates, or any portion of the profits thereof. The undersigned Bidder agrees that he will complete the Work in all respects as required by I Division C, Article 2. Time for Completion and that he will pay liquidated damages to the City in the amount specified in the Contract. Accompanying this Proposal is a bid bond or certified check in the amount of five percent(5%)of I the Proposal according to the conditions of the"Notice to Contractors"and"Division 1 —General Requirements" hereby attached. If this Proposal shall be accepted by the City of Everett, Washington, and the undersigned shall fail to execute a satisfactory contract and bond,as stated in I the Division 1 — General Requirements hereto attached, within 14 calendar days after the Award Date,then the City may, at its option, determine that the undersigned has abandoned the Contract and thereupon this Contract shall be null and void and the amount of the bid bond or certified check accompanying this Proposal shall be forfeited and become the property of the City of IEverett, Washington.
