March 8, 2012
PUBL/C ft/OR/C5
VERIZON NORTHWEST INC
PO BOX 152206
IRVING TX 75015
Subject: Additional Backflow Prevention Requirements for City of Everett site address:
2604 RUCKEFELLER AVE
Dear Property Owner:
The protection of the City of Everett water system from potential private customer hazards is a
Washington State Department of Heaith (DH) requirement and you are being notified by the City of
Everett, the water purveyor for the building address listed in the subject line above, that construction
upgrades are required for your property when there are modifications required to the sprinkler system,
based on fu[ure construction or change of use. It is important to us and to our customers that the public
water system delivers high quality potable water in all cases.
City of Everett records indicate tliat this building is served by a 6-inch single check-valve. A double check
detector is required by the State of Washington for commercial fire services. To comply with state
requirements, the city requires the upgrade of the single check-valve to a double check detector
assembly when the commercial building sprinkler sys:m is modified through the city permit process.
Please be advised that the city requires this upgrade as a part of building or sprinkler permit application
(or the portion of fhe building served by the wrrent single check valve.
As the owner of this assembly, you are required to maintain it and improve it as necessary. Following is
the list of contacts lhat you can use to darify how this requirement to upgrede to a DCDA will impact
your property:
If you have questions regarding building or sprinkler permit requirements and the City of
Everetl Design and Construction Standards for the location of the DCDA (vault or
interior), please contact Meg Haley by e-mail at mhalev ci.everett.wa.u or by phone
at425-257-8813.
If you have questions regarding the fire service and coordination of the new DCDA with
the sprinkier system and Fire Department requirements, please contact Assistant Fire
Marshal 7im Venturo by e-mail at jventuroCci.everett.wa.us or by phone at 425-257-
8124.
.
