The City of Everett has not <br /> received this form yet. Please have your backflow prevention assembly tested and return the <br /> report to us by 05/04/17. <br /> The annual test is required under the State Department of Health Washington Administrative <br /> Code 246-290-490 and the City of Everett Municipal Code Chapter(EMC) 14.16.470. Both the <br /> EMC and state law require the owner of the assembly to perform a successful test every year by a <br /> state certified backflow prevention assembly tester and submit the results to us.
