The following information is still needed prior to further plan review. <br />1. No manufacture name or model number was submitted for the dishwasher, item #17 on the <br />equipment list. This information must be submitted. <br />2. No model number was submitted for the True pizza preparation table item, #22 on the <br />equipment list. This information must be submitted. <br />3. An ice machine is indicated on the floor plan in the back room next to the handwash sink. It <br />is not included on the equipment list. The manufac!urer name and model number for the ice <br />machine must be submitted. <br />4. Fartners Brothers coffee equipment is indicated on the Floor plan at the wait stntion. It is not <br />included on the equipment lict. The manufacturer name and model number for the coffee <br />eyuipment must be submitted. <br />5. All food service equipment must be listed by the Nutional Sanitation Foundation (NSF) for <br />its intended use. The True refrigerator model #GTM-d7 PE57119 (item #7, #8, #9 and #10), <br />was not found in the current NSF or equivalent listing. This unit must be replaced with NSF <br />or equivalent listed equipment or documentation must be submitted which demonstrates NSF <br />equivalency. Manufacturer names and model numbers for replacement units must be <br />submitted. It is recommended that the original model number be checked for accuracy. Did <br />you perhaps mean GDM instead of GTM? <br />6. No handwash sink is shown at the wait station. A hundwash sink is requircd at thc wait <br />station. <br />Please note that priar to upenin� oF the new facility. after the Health District plan reciew process <br />is completed and construction is finished a pre-operational inspection must be conducred. <br />Please cont�ct me if you have any questions.
