B. The Contractor is required also to document such effort on the "City of Everett Disadvantaged/Women's <br /> Business Enterprise" form and to submit said form, in accordance with Document 00 45 39 of the <br /> Contract Documents,with its Bid. <br /> 1.29 NON-COLLUSION AFFIDAVIT <br /> A. The Contractor must submit a"Non-Collusion Affidavit" in accordance with Document 00 45 19 of the <br /> Contract Documents as part of the Bid package. <br /> B. If the City has reason to believe that collusion exists among Bidders, the City will reject the Bids of the <br /> known participants in such collusion and may, at its option, require that all Bidders certify under penalty <br /> of perjury,that no collusion has occurred or exists. <br /> 1.30 STATE SALES TAX <br /> A. The Washington State Department of Revenue has issued special rules on the State sales tax. I <br /> B. The Contractor should contact the Washington State Department of Revenue for answers to questions in <br /> this area. I <br /> C. The City will not adjust its payment if the Contractor bases a Bid on a misunderstood tax liability. <br /> D. The Contractor shall not include State retail sales taxes in the unit bid prices. <br /> 1. A separate line item for applying State retail sales tax is provided in the Bid Form. <br /> 1.31 LIMITATIONS REGARDING CONTRACTOR'S CLAIMS FOR DAMAGES <br /> A. Bidders should review the Contract Documents regarding limitations on claims for damages. 1 <br /> 1.32 DELAYS AND INTERFERENCE <br /> A. Bidders should review the Contract Documents regarding delays and interference. <br /> 1.33 BIDDER'S CHECK LIST <br /> A. The Bidder's attention is especially called to the following forms that must be executed in full as required <br /> and submitted with the Bid. <br /> 1. Bid: Show the lump sum and unit price items in the space provided on the Bid Form. <br /> 2. Bid Signature Sheet: To be filled in and signed by the Bidder. <br /> 3. Subcontractors Form: If required,to be filled in by the Bidder. <br /> 4. Bid Security: This form is to be executed by the Bidder and the surety company unless Bid is <br /> accompanied by a certified or cashier's check. The amount of the bond or cashier's check, which <br /> shall be not less than five percent of the Total Bid Amount bid, may be shown in dollars or on a <br /> percentage basis. I <br /> 5.
