Cost Estimate for Permit Fee Calculation Form: (1) copy of cost estimate form filled out for <br /> the City to use to calculate the Public Works Permit Fee. <br /> vi. Easement Form: (1) copy of easement form for public access easement on Fleming St with <br /> legal description and exhibit attached for review.
