EVERETT <br /> WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2019 through December 31, 2019 <br /> Exhibit A <br /> Program Summary <br /> Organization&Program: Cocoon House: Cocoon House Central: Emergency Teen Shelter <br /> Cocoon House Central Shelter is an eight(8)bed residential facility providing temporary housing and supportive <br /> services to homeless and at-risk youth.The program is licensed to serve youth ages 12 through 17. Cocoon House <br /> staff work one-on-one with each youth to create and implement an individualized care plan focused on safe housing <br /> (through family reunification whenever possible),strengthening a youth's connection to school,and building skills <br /> for self-sufficiency.In PY 2019,Cocoon House Central will serve 80 youth,at least 40 of those being Everett <br /> residents. <br /> Cocoon House works with homeless and at-risk youth and their families to break the cycle of youth homelessness. <br /> The specific objectives of Cocoon House Central are to: <br /> -Reduce youth homelessness by providing short-term emergency housing <br /> -Provide youth and families the necessary support to attain family reunification or other stable housing <br /> -Provide an environment that fosters self-sufficiency and growth in independent living skills <br /> -Develop or strengthen a youth's involvement in school(a key protective factor against future homelessness) <br /> -Improve youth well-being through physical and behavioral health services <br /> Specific Outputs and Outcomes: <br /> -At least forty(40)unduplicated youth from Everett receive housing and supportive services <br /> -At least 500 bed nights are provided(with 62 of those funded by City of Everett Human Needs) <br /> -At least seventy-five percent(75%)of youth will exit directly to safe housing with priority on family reunification <br /> -At least seventy-five percent(75%)of youth have a positive school status at the time of exit <br /> -At least eighty-five percent(85%)of youth will demonstrate independent living skills while in the program <br /> 2019 Human Needs Contracts 7 <br />
