EVERETT <br /> WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2019 through December 31, 2019 <br /> Exhibit A <br /> Program Summary <br /> Organization&Program: Cocoon House:Outreach Services/U-Turn Drop In Center <br /> Cocoon House Street Outreach and U-Turn Drop In Center serve homeless and at-risk youth ages 12 through 24 <br /> with the goal of reducing youth homelessness and assisting youth in improving self-sufficiency.The Outreach <br /> programs provide case management,mobile and stationary outreach,meals and basic needs items, supported <br /> connections to other providers,and other services and referrals. <br /> Cocoon House works with homeless youth and their families to break the cycle of youth homelessness. Specific <br /> Outreach objectives include: <br /> -Increased safe access to basic needs <br /> -Increased access to safe housing and other resources <br /> -Improved health and behavioral health status <br /> -Improved independent living skills and permanent supports <br /> -Reduction in violent of delinquent behaviors <br /> Measurable Outputs/Outcomes <br /> -At least 400 Everett residents engage in Outreach services(356 funded by this grant) <br /> -At least 8,000 units of basic needs items are distributed(356 funded by this grant) <br /> -At least 60%of youth engaged in case management make progress on safe housing goals <br /> -At least 75%of youth engaged in case management make progress on positive youth development goals(including <br /> education,employment,life skills,and health goals) <br /> 2019 Human Needs Contracts 7 <br />
