B. The applicant' s age, marital status , height, color <br /> of eyes , and color of hair. <br /> C. Whether he has ever been convicted of a felony or <br /> misdemeanor, and if so, when and where and for what <br /> cause. <br /> D. The applicant's training and experience in the <br /> transportation and care of patients, and whether he <br /> has previously been licensed as a driver, chauffeur, <br /> emergency medical technician or conditional driver, <br /> and if so, when and where, and whether his license has <br /> ever been revoked or suspended in any jurisdiction and <br /> for what cause. <br /> E. Affidavits of good character from two reputable <br /> persons who have personally known such applicant and <br /> observed his conduct during three years next preceding <br /> the date of his application. <br /> F. Two recent photographs of the applicant, of a size <br /> designated by the License Officer, one of which shall <br /> be attached by the License Officer to the license. <br /> G. Such other information as the License Officer shall <br /> deem reasonably necessary to a fair determination of <br /> compliance with this Ordinance. <br /> H. An accompanying license fee of $5 .00 for each indi- <br /> vidual applicant for an Ambulance Attendant or Conditional <br /> Ambulance Attendant License (in addition to license fee <br /> for Ambulance Operator/Emergency First Aid Service Opera- <br /> tor 's License) . <br /> I. Employees of the City of Everett who perform ser- <br /> vices the same as or similar to an Ambulance Attendant <br /> pursuant to their official duties in the course of their <br /> employment with the City of Everett shall not be obligated <br /> to pay this license fee, but must conform to all other <br /> regulations as set forth by this Ordinance . <br /> Section 9 : Standards for Ambulance Attendant/Emergency <br /> Medical Technician and Conditional Ambulance Attendant/ <br /> Emergency Medical Technician License <br /> A. Ambulance Attendant/Emergency Medical Technician - <br /> Each ambulance operator and emergency first aid service <br /> shall have, for each vehicle in service, on duty and <br /> available for immediate response, two Ambulance Attendants/ <br /> Emergency Medical Technicians who are licensed for emer- <br /> gency medical service as provided in Section 9B of this <br /> Ordinance. A certificate of license shall be carried on <br /> the person of each ambulance attendant/emergency medical <br /> technician and conditional ambulance attendant/emergency <br /> medical technician while on duty. <br /> B. Ambulance Attendant/Emergency Medical Technician - <br /> Certificate of license - no person shall perform the <br /> duties of an Ambulance Attendant/Emergency Medical Techni- <br /> cian without first obtaining an Emergency Medical Techni- <br /> cian 's Certificate. No such Ambulance Attendant/Emergency <br /> Medical Technician license shall be issued to any appli- <br /> cant who has not previously held such an EMT Certificate, <br /> or whose Emergency Medical Technician Certificate has <br /> been expired for more than one year, unless : <br /> -7- <br />
