B. The applicant' s age, marital status , height, color of eyes , and color of hair. C. Whether he has ever been convicted of a felony or misdemeanor, and if so, when and where and for what cause. D. The applicant's training and experience in the transportation and care of patients, and whether he has previously been licensed as a driver, chauffeur, emergency medical technician or conditional driver, and if so, when and where, and whether his license has ever been revoked or suspended in any jurisdiction and for what cause. E. Affidavits of good character from two reputable persons who have personally known such applicant and observed his conduct during three years next preceding the date of his application. F. Two recent photographs of the applicant, of a size designated by the License Officer, one of which shall be attached by the License Officer to the license. G. Such other information as the License Officer shall deem reasonably necessary to a fair determination of compliance with this Ordinance. H. An accompanying license fee of $5 .00 for each indi- vidual applicant for an Ambulance Attendant or Conditional Ambulance Attendant License (in addition to license fee for Ambulance Operator/Emergency First Aid Service Opera- tor 's License) . I. Employees of the City of Everett who perform ser- vices the same as or similar to an Ambulance Attendant pursuant to their official duties in the course of their employment with the City of Everett shall not be obligated to pay this license fee, but must conform to all other regulations as set forth by this Ordinance . Section 9 : Standards for Ambulance Attendant/Emergency Medical Technician and Conditional Ambulance Attendant/ Emergency Medical Technician License A. Ambulance Attendant/Emergency Medical Technician - Each ambulance operator and emergency first aid service shall have, for each vehicle in service, on duty and available for immediate response, two Ambulance Attendants/ Emergency Medical Technicians who are licensed for emer- gency medical service as provided in Section 9B of this Ordinance. A certificate of license shall be carried on the person of each ambulance attendant/emergency medical technician and conditional ambulance attendant/emergency medical technician while on duty. B. Ambulance Attendant/Emergency Medical Technician - Certificate of license - no person shall perform the duties of an Ambulance Attendant/Emergency Medical Techni- cian without first obtaining an Emergency Medical Techni- cian 's Certificate. No such Ambulance Attendant/Emergency Medical Technician license shall be issued to any appli- cant who has not previously held such an EMT Certificate, or whose Emergency Medical Technician Certificate has been expired for more than one year, unless : -7-
