EVERETT <br /> WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2019 through December 31, 2019 <br /> Exhibit A <br /> Program Summary <br /> Organization &Program: Friends of Youth: New Ground Everett Transitional Living <br /> New Ground Everett supports 12 residents in six two-bedroom apartments in the Silver Lake neighborhood of <br /> Everett.The program provides a safe home and the support needed for young adults to develop independent living <br /> skills,pursue education and employment goals,increase social skills,manage life's stresses,and successfully move <br /> into stable housing. <br /> When residents first arrive,we focus on helping them adapt to their new home and new routines while building trust <br /> with program staff,roommates and neighbors. We then use evidence-based screening tools,including the Arizona <br /> Self Sufficiency Matrix and the Ansell-Casey Life Skills Assessment,to assess each individual's needs and <br /> strengths.The results of these assessments form the basis for discussions between the Case Manager and the client <br /> to develop an Independent Living Plan(ILP). The ILP focuses on achievable,time-bound goals regarding <br /> permanent housing,employment,life skills and personal finances. The residents are the decision makers concerning <br /> their goals and work closely with the Case Manager to adjust goals and action steps as needed through weekly <br /> meetings. <br /> Many aspects of the program play a role in establishing healthy routines and behaviours, including: <br /> •Service learning(volunteer)activities provide opportunities to learn how to have a positive impact and make <br /> personal connections with supportive adults. <br /> •Encouragement and support guide the young adults to re-build relationships with their families and establish <br /> positive relationships with their peers. <br /> •Referrals help the young adults address substance use,mental health concerns,GED completion and job training. <br /> Life skills learned include budgeting,grocery shopping,basic cooking,household maintenance,building appropriate <br /> relationships and healthy decision-making.Additionally,all residents attend monthly house meetings where they <br /> build a sense of community and strengthen interpersonal skills.At these meetings,the Case Manager facilitates <br /> group discussions to help residents improve emotional and self-regulating coping skills.Topics include:respecting <br /> neighbors and roommates;recognizing personal needs for space or time alone;and the importance of self-care <br /> (including healthy eating,getting enough sleep,walks and talking through their problems with their Case Manager.) <br /> 2019 Human Needs Contracts 7 <br />
