Computer Sciences Corporation <br /> Work Order <br /> June 7,2019—Prices Valid Until June 30, 2019 <br /> Work Order No.3 <br /> This Work Order is effective May 8,2019 and is hereby made a part of and incorporated into the into Agreement for Purchase, <br /> Installation and Training of Risk Management Information System by and between Computer Sciences Corporation <br /> ("CSC") and City of Everett ("Customer") dated as of March 31, 2000 and any amendments thereto (the "Agreement"). <br /> This Work Order is governed by the terms and conditions of the Agreement.CSC and CUSTOMER hereby agree as follows: <br /> SERVICES <br /> Training: <br /> Estimated <br /> Description No.of days Daily Rate <br /> RISKMASTER Accelerator Training 2 $2200.00 <br /> Includes Training Project Management <br /> Estimated <br /> Description No.of hours Hourly Rate <br /> Database Analysis 20 $ 132/hr. <br /> Project Management 15 $ 132/hr. <br /> ASSUMPTIONS: <br /> • Assumes client is moving to AWS multi-tenant hosting from Riskmaster Accelerator eXpress <br /> • Customer will be upgraded to multi-tenancy Riskmaster in AWS on the most recent version available on a Multi- <br /> Tenant platform <br /> • Customer requires a Database analysis from version 16.3 or prior. <br /> • Additional project management is required for this upgrade. <br /> • Should additional hours be required once databse analysis is completed,a project change control will be required for <br /> additional hours. <br /> • Business Intelligence Training is out of scope. <br /> • Project Management associated with this work order is limited to the contracted training: scheduling resource,chairing <br /> pre-training call,providing customized agenda and post-training follow-up. <br /> Database Analysis Assumptions: <br /> • Customer will share/provide a copy of their RISKMASTER Production database,View database, Session database and <br /> Security database. <br /> Note 1: CUSTOMER will pay all reasonable travel-related expenses incurred by employees of CSC in connection with this <br /> project if client elects for onsite training.
