MONTHLY APPRENTICE UTILIZATION REPORT 5.1 With each Request for Progress Payment, the Contractor shall submit to City a Monthly Apprentice Utilization Report in an electronic format to be provided by City, for the Contractor and all subcontractors. The Contractor and all subcontractors performing ' work on the Project during the reporting period shall complete and submit the Monthly Apprentice Utilization Report. The report shall accompany the Contractor's Request for Progress Payment. A Request for Progress Payment that is not accompanied by the Monthly Apprenticeship Utilization Report will not be processed for payment until said Monthly Apprenticeship Utilization Report is received and approved by City. 5.2 The Contractor shall be responsible for reporting apprentice utilization data required by ' City beginning with the first day of work for each apprentice. The Contractor's first submittals are due at the end of the first month after the Contract start date specified in the Notice to Proceed, and at monthly intervals thereafter as specified above. ' Subcontractor submittals are due at the end of the month after commencement of their work and monthly thereafter, as specified above. 5.3 The Contractor shall report the following information on each apprentice: ' 5.3.1 Apprentice's Name 5.3.2 Home Zip Code 5.3.3 Employment Status: New Hire or Existing Staff ' 5.3.4 Trade/Craft i5.3.5 State Apprentice Registration I.D.
