City of Everett 00 6800 - 3 <br /> 2019 Biosolids Removal UT3705-2 <br /> 5. MONTHLY APPRENTICE UTILIZATION REPORT <br /> 5.1 With each Request for Progress Payment, the Contractor shall submit to City a Monthly <br /> Apprentice Utilization Report in an electronic format to be provided by City, for the <br /> Contractor and all subcontractors. The Contractor and all subcontractors performing <br /> ' work on the Project during the reporting period shall complete and submit the Monthly <br /> Apprentice Utilization Report. The report shall accompany the Contractor's Request for <br /> Progress Payment. A Request for Progress Payment that is not accompanied by the <br /> Monthly Apprenticeship Utilization Report will not be processed for payment until said <br /> Monthly Apprenticeship Utilization Report is received and approved by City. <br /> 5.2 The Contractor shall be responsible for reporting apprentice utilization data required by <br /> ' City beginning with the first day of work for each apprentice. The Contractor's first <br /> submittals are due at the end of the first month after the Contract start date specified in <br /> the Notice to Proceed, and at monthly intervals thereafter as specified above. <br /> ' Subcontractor submittals are due at the end of the month after commencement of their <br /> work and monthly thereafter, as specified above. <br /> 5.3 The Contractor shall report the following information on each apprentice: <br /> ' 5.3.1 Apprentice's Name <br /> 5.3.2 Home Zip Code <br /> 5.3.3 Employment Status: New Hire or Existing Staff <br /> ' 5.3.4 Trade/Craft <br /> i5.3.5 State Apprentice Registration I.D. Number <br /> ' 5.3.6 Program Sponsor and/or Hiring Source <br /> 5.3.7 Apprentice Progression Period or Percentage <br /> 5.3.8 Ethnicity/Gender <br /> 111 5.3.9 Labor hours for Reporting Period by Ethnicity/Gender <br /> 5.4 The Contractor shall report the following information on the total number of apprentices: <br /> 5.4.1 Total labor hours and number of apprentice employees for reporting period by <br /> thriiritvfrinrfr. <br /> 5.4.2 Total apprentice labor hours and number of employees to-date. <br /> 00 6800 - 3 APPRENTICE UTILIZATION AND REPORTING <br />
