t ' <br /> 12011 19th Avenue SE <br /> Everett,WA 98028 <br /> Project Number:2018-0363 <br /> 1.0 SCOPE OF WORK <br /> A Good Faith Asbestos Inspection was conducted at the "ex-fire house" located at 12011 19th Avenue <br /> SE, Everett, WA 98028 on May 10, 2018. <br /> Jason Lindahl (an AHERA Certified Building Inspector), conducted this inspection at the request of Mr. <br /> Emory Cole of Emory's Lake house, LLC. <br /> The purpose of this inspection was to identify asbestos containing building materials which would be <br /> impacted by the planned demolition of the commercial building. Destructive sampling methods were <br /> utilized to collect samples of suspect building materials. No soft/limited demolition was performed during <br /> this inspection. Please note that hidden materials may exist within the structure, and all suspect <br /> materials must be treated as asbestos containing until testing proves otherwise. <br /> This survey constitutes a survey of accessible suspect ACM in the project area and was conducted in <br /> accordance with: <br /> The National Emission Standards for Hazardous Air Pollutants (NESHAP) 40 Code of Federal <br /> Regulations (CFR) Part 61, Subpart M requires a survey by an accredited asbestos inspector prior to <br /> demolition of a structure. <br /> This asbestos survey also satisfies the requirements for"Good Faith" inspection outlined in Washington <br /> Administrative Code (WAC) 296-62-07221(2), Identification, which requires the owner of a structure to <br /> provide contractors with a written report identifying the asbestos-containing materials expected to be <br /> disturbed during renovation or demolition. <br /> The asbestos survey section is written to comply with the AHERA asbestos sampling procedure as <br /> stated in 40 CFR 763.86. This protocol is required under the Puget Sound Clean Air Agency (PSCAA <br /> Regulation III, Article IV, rev.
