EVERETT <br /> MIMI WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2019 through December 31, 2019 <br /> Exhibit A <br /> Program Summary <br /> Organization & Program: Housing Hope: Homeless Teen and Young Parent Program <br /> Housing Hope's Homeless Teen and Young Parent Program provides affordable housing and comprehensive <br /> services to homeless teen parents ages 16 to 24 and their young children. Services include Family Support Services <br /> (formerly called case management),adult education and employment training,parent education and support, life <br /> skills training,child care,and supported access to critical community services such as mental health and substance <br /> abuse treatment. This project will serve approximately 29 teen parent families in Everett(approximately 75 <br /> individuals),all of whom are homeless at entry. The overarching goals of this program are to assist these very young <br /> parents to complete necessary adolescent developmental tasks,to heal from past abuse/neglect and to build a solid <br /> foundation of education, employment,parenting and independent living skills that will lead to permanent housing <br /> and self-sufficiency. <br /> Project objectives for this program are: <br /> 1. Assist homeless teen parents in maintaining their housing or moving to other permanent housing in the <br /> community. <br /> 2. Assist homeless teen parents in increasing their earning potential, including completing high school or a GED <br /> program,advanced educational program or job training program. <br /> 3. Assist homeless teen parents in increasing their total houshold income, including earned income from <br /> employment and public benefits as appropriate and elgible. <br /> 4. Assist homeless teen parents in increasing their household management skills, including parenting skills that <br /> will ensure healthy child development. <br /> Program Components and Structure: <br /> Housing Hope offers a comprehensive program designed to assist these very young homeless families to gain the <br /> competencies, assets and confidence to transition to their adult roles and responsibilities,maintain permanent <br /> housing and increase levels of self-sufficiency. Family Support Coaches in this project focus on helping these <br /> parents complete their adolescent development,complete their basic education, establish a strong foundation of <br /> employment, acquire life skills, and parent their very young children. The primarily role of the Family Support <br /> Coach is to provide a trusting,consistent and supportive adult relationship that becomes the launching point from <br /> which these teens build connections with the community and gain the confidence to view themselves as capable <br /> adults. Family Support Coaches support these young parents to create a vision for their adult lives and to develop an <br /> Individualized Family Plan,outlining the specific long and short-term goals for permanent housing and self- <br /> sufficiency. Through in-home services and connections to community services, Family Support Coaches help <br /> families learn new skills,change attitudes, and complete activities that move them progressively towards the <br /> achievement of their long-term goals. Services are tailored(type, frequency,and amount)to meet the unique needs <br /> of each family. <br /> Adult Education and Employment Services are offered on-site and in the community for all participants of this <br /> program. Housing Hope employs nine Education and Employment Specialists, who provide one-on-one counseling <br /> and support to assist parents in the development of educational and employment skills. Employment offers these <br /> teens the opportunity to increase their sense of self-worth and is critical to sustaining permanent housing and <br /> increased levels of self-sufficiency. A continuum of employment services assist participants to move to job <br /> readiness and then towards a career with a livable wage. Housing Hope collaborates with WorkSource, Department <br /> of Social and Health Services WorkFirst, Workforce Snohomish, Department of Vocational Rehabilitation, Everett <br /> 2019 Human Needs Contracts 7 <br />
