om EVERETT <br /> WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2019 through December 31, 2019 <br /> Exhibit A <br /> Program Summary <br /> Organization&Program: Catholic Community Services of Western Washington for CHART Case Management <br /> Provide housing support services to twenty(20)extremely low-income, Literally Homeless and at Imminent Risk of <br /> Homelessness households selected from the CHART List by the City of Everett.The CHART case manager will primarily focus <br /> on priority needs of the identified CHART individuals to help reduce contacts with criminal justice and emergency response <br /> systems.The primary intervention is facilitating necessary services that will support an individual to reside indoors.The <br /> expected roles and activities of the case manager will include but are not limited to: <br /> • Act as an essential member of a multi-disciplinary team providing on-going case management services with a harm- <br /> reduction focus to adults with severe and persistent mental illnesses and drug and alcohol-related issues. <br /> • Provide outreach and engagement services to assigned clients,to initially engage clients and maintain working <br /> relationships. <br /> • Conduct psychosocial assessments; develop goal planners with consumer participation;facilitate linkages to <br /> collaborative resources when appropriate. <br /> • Assume primary responsibility for coordinating all aspects of consumers' support and goal planners.This includes: <br /> providing and/or coordinating the delivery of psychiatric treatment;substance abuse treatment;and performing <br /> acute case management to facilitate consumer access to food, clothing, housing, medical care,financial assistance and <br /> vocational training or employment.Work with other services providers to provide robust support and a plan for off <br /> hour needs of the client. <br /> • Advocate for consumers' access to community resources and services,ensuring that consumers' needs are met and <br /> rights are maintained;consult and collaborate with community providers to ensure continuity of care. <br /> • Transport clients by providing or arranging transportation to necessary appointments that meet client goals. <br /> • Participate in the planning, organizing and facilitating of weekly CHART meetings presenting updates on all active <br /> clients. <br /> • Travel locally to outreach consumers; meet them on the street/in the community, make home visits,and assist with <br /> home inspections. <br /> • Provide,at times, crisis intervention and risk assessment,which could require tolerating unusual or unpleasant <br /> behaviors. <br /> • Document services provided. <br /> • Plan,structure&support community based activities such as obtaining and setting up housing,grocery shopping, <br /> nutrition assistance, house cleaning and other household activities, money management, social activities and work- <br /> related support such as job coaching. <br /> • Coordinate with outside service providers including inpatient&outpatient treatment services,state vocational <br /> rehabilitation services,social security,veteran benefits, legal &advocacy services and personal primary care: medical <br /> and dental providers. <br /> • Provide practical help and supports, advocacy,coordination,side-by-side individualized support, problem-solving, <br /> direct assistance,training, and supervision to help participants obtain the financial support such as entitlements or <br /> housing subsidies; money management services(e.g. payeeships);and transportation. <br /> • Coordinate with the YWCA,who administers the HOME Tenant Based Rental Assistance funds for CHART individuals. <br /> • The program will use a Low-Barrier and Housing First approach focused on consumer centered goals and will also <br /> work with landlords at scattered-site locations to integrate individuals into housing and to remediate any damage. <br /> • Visit clients who are incarcerated,which may include passing a background screening. <br /> • Case managers will track data and report outcomes to the CHART related to housing status and psycho-social well- <br /> being as measured by an agreed upon assessment tool and timeframes. <br /> 7 <br />
