Md7 <br />July 9, 2018 <br />To: <br />City of Everett, Permit Services <br />3200 Cedar St., 2"d Floor <br />Everett, WA 98201 <br />From: <br />Brent Sutten <br />9725 3RD Ave. NE, Suite 410 <br />Seattle, WA 98115 <br />bsutten@md7.com <br />Re: Site ID SE01S123A — MOSQUITO FLEET HOSPITAL <br />Hello, <br />Please find the following documents enclosed for the Building Permit application for T-Mobile to <br />upgrade its existing wireless communication facility located at 1321 Colby Ave, Everett, WA 98201 <br />(Parcel #00438524600000). <br />Description of Work: The scope of work for this site is as follows: <br />Keep All Existing COVP-Swap COVP Breakers to Duals; No Change On Antennas, Replace 3 FXFCS with 3 <br />FHFBS for 1-1900; Keep All Diplexers, Keep All FRUS For L2100, Keep All FRLBS For 1-700; Remove GSM; <br />Remove ESMB; Keep Existing FSMF for 1-2100; Replace Other Existing FSMF and FBBCS Used For LTE with <br />1 ASIA and 2 ABIA; Add 1 AMOB; Replace (2) Ultrasite Cabinets W/ (1) FCOA Cabinet for AMOB Install. <br />6409 Eligibility: This project includes the replacement of old, and addition of new transmission <br />equipment on an existing base station. The proposed work will not increase the height or horizonal <br />profile of the base station and will not add any new cabinets of equipment. As such, it qualifies as an <br />eligible facility request under Section 6409(a) of the Middle Class Tax Relief and Job Creation Act of <br />2012. <br />Please find the following documents enclosed with this application: <br />1.
