Everett <br />Grant Program <br />Everett CARES Round Three <br />SUPPLEMENTAL Small Business Grant Agreement <br />RECITALS <br />This Agreement is entered into upon the basis of the following facts and circumstances: <br />1. <br />City: <br />The City of Everett, which is hereafter referred <br />to as "City" or "Grantor" <br />2. <br />Business Applicant (herein referred to as <br />"Business" or "Grantee"): <br />EVT Music Initiative <br />3. <br />Address: <br />3608 Norton Ave. Everett, WA 98201 <br />4. <br />Phone: <br />2066790848 <br />5. <br />Contact Person: <br />Ryan Crowther <br />6. <br />DUNS Number <br />122949850 <br />7. <br />Title of Program being Funded: <br />Everett CARES Round 3 SUPPLEMENTAL Small <br />Business Grant <br />8. <br />Time of Performance: <br />Beginning: <br />Ending: <br />August 1, 2020 <br />July 31, 2021 <br />9. <br />Amount of Agreement/Grant Award: <br />$3,000 <br />10. <br />This Agreement/grant award and the rights and <br />obligations of both parties hereto shall be subject <br />to and governed by the following, incorporated by <br />reference herein as is fully set forth: <br />(1) Grant Budget attached hereto or Exhibit "A" <br />(2) Expense Reporting Form attached hereto or <br />Exhibit "B" <br />The City has funds available, authorized by the City Council for helping businesses respond to the negative economic impact of <br />the Coronavirus; and <br />The Business has been awarded assistance from the Everett CARES Rounds 1 or 2 Small Business Grant Program; and relying <br />on the information provided by the Business, the City has determined that Business is a small business at risk of long-term business <br />injury or closure as a result of the Coronavirus; and <br />The Business wishes to receive a grant from the City in the sum of Three Thousand ($3,000) to be used as working capital to <br />retain jobs, to expand commerce, and to off -set the negative impact of the coronavirus on its business; and <br />2 <br />
