CHANGE ORDER # Project (Procurement) Number: Small Works Roster Project Title: IFQ #2022-066 Madison Morgan – Park Trail Improvements Description of Change: BUILDING/LOCATION: Madison Morgan Park, 518 Madison Street, Everett, WA 98203 The monument sign base has been revised from a 6” thick concrete base to a 9” thick concrete base. This change order covers the additional cost of framing material and concrete. The original drawings specified that the monument sign required a 6” thick concrete base. The contractor indicated that the work for the monument sign base was to begin, and the project manager informed Permit Services and provided the proposed drawing. Upon inspection of the drawing by Permit Services, Permit Services requested that the base be increased from 6” thick to 9” thick to comply with city signage standards.
