Dogs Playing for Life Seminar Agreement <br />This Agreement is made and entered in to as of date of signature below <br />between and among Dogs Playing for Life (DPFL) and City of Everett (Host <br />Shelter). <br />Terms of Agreement <br />The Host Shelter's Dogs Playing for Life™ Seminar is scheduled to take place at the Everett Animal <br />Shelter, 333 Smith Island Road, Everett, WA 98201 for a total of (3) three days. <br />The Host Shelter shall comply with the following: <br />Host Shelter commit to the dates and times provided by Dogs Playing for Life and will not cancel or <br />reschedule without proper and timely notification. Specifically, if Host Shelter cancels within four (4) <br />weeks (i.e., 28 days) of the scheduled and confirmed dates, the Host Shelter will be responsible for <br />all costs incurred by DPFL with regard to cancelling staff and travel plans. <br />• No decisions regarding a dog's suitability for play group participation are made based on the <br />dog's suspected breed or breed mix (all Recipient Host Shelter dogs are treated individually). <br />•The Host Shelter must participate in the DPFL's follow-up survey. <br />•The Host Shelter will provide all data requested by DPFL prior to the seminar, and at <br />intermittent follow up periods as designated by DPFL. In exchange, the shelter may be invited to <br />apply for a grant through the Lazin Animal Foundation, which sponsors data collection and <br />follow up. <br />To the extent of Host Shelter’s negligence, the Host Shelter agrees to hold Dogs Playing for Life™, their <br />officers, employees, successors and assigns, harmless from any cost or expense, including reasonable <br />attorney fees, that may result from any claims, suits, actions and proceedings filed or brought in <br />connection with or as a result of Host Shelter s activities. <br />Should the Host Shelter fail or neglect, in the reasonable judgment of Dogs Playing for Life, to meet some <br />or all of the provisions above, Dogs Playing for Life shall have the right to terminate agreement upon <br />timely written notice to the Host Shelter setting forth the purported breaches of the Host Shelter.
