This Change Order is intended to be made a part of that certain Schedule of Protection, Proposal and Sales Agreement <br />("Agreement") between Everon LLC, ("Everon") and the Customer listed above for service at the location(s) listed below. <br />The Customer hereby agrees to pay Everon, its Agents or Assigns, the sum of $34.98 plus any applicable taxes. In addition, <br />Customer agrees to pay Quarterly in advance the additional sum of $0.00.
