<br /> <br />CHANGE ORDER/AMENDMENT <br /># <br /> <br />Contract (Procurement) Number: Services Agreement Contract Title: <br />RFP #2024-020 Bus Schedules Printing <br />Description of Change: <br />1. As allowed per the Services Agreement Extension Provision, the City extends the contract for one <br />year. This shall be the 3rd contractual year (2nd option period), during which the renewal term will be <br />from 6/7/2026 to 6/7/2027. <br />2. As allowed by Section 2.4 Changes in Scope of Work of the RFP, pricing is added for 12,500 <br />finished booklet size of 80 inside pages plus a 6-page cover for $8,840.86. <br />3. Base year pricing shall remain unchanged, as shown in the original proposal submitted. <br />Regardless of the date(s) on which this Change Order/Amendment is signed by the parties, and regardless of any <br />Agreement completion date(s) that may have been in the Agreement prior to this Amendment, the parties agree that the <br />Agreement is deemed continuously in effect since the Original Agreement Date. <br /> <br />All provisions in the Agreement shall remain in effect except as expressly modified by this Change Order/Amendment. <br />From and after the effective date of this Change Order/Amendment, all references to the Agreement in the Agreement <br />are deemed references to the Agreement as modified by this Amendment. <br /> <br />By signing below, City of Everett and the Service Provider agree that the Agreement shall be changed to the extent <br />described in the above “Description of Change.” This Change Order/Amendment only changes the Agreement between <br />City of Everett and the Service Provider to the extent explicitly provided in this Change Order/Amendment.
