Last weekend the Fire Department located an illegally occupied hookah lounge (smoking <br /> lounge per the IMC) in the basement of 2701 Wetmore Ave when a fire alarm was tripped. Ted Budden is <br /> working with Kevin Fagerstrom to create a new Code Violation case. <br /> 1. Please create a new W complaint record in TRAKiT for 'OCCUPANCY WITHOUT PERMITS' subtype <br /> COMPLAINT and status ACTIVE. The inspection record to be attached to the W record is from the Fire <br /> Department: Report attached. <br /> 2. IF you agree, then, the following permits (currently in review) would then be changed to sub-permits <br /> with a W designation under the new 'W' record: <br /> a. Smoking lounge, permits B1301-007 and M1201-037; and <br /> b. Change of occupancy permit C1110-003 for the second floor offices conversion to <br /> residences. <br /> 3. IF you agree, then, the following issued permit would then be moved to as a sub-permit under the new <br /> 'W' record: <br /> a. Automatic suppression and detection for the building, permit K1207-009. <br /> Please make new labels for item '2', and then I can update the folders and drawings that are in review with <br /> Steve Miller and myself. The occupancy is identified as commercial in both County tax and City permit. <br /> 2. Previous code violations have been closed. This is a request to start a new code violation case. <br /> 3. The basement is being used as a hookah bar (smoking lounge) as reported by Captain Brent Stainer of <br /> the Everett Fire Department on January 15, 2013. Report attached. <br /> 4. A smoking lounge requires permits, approvals, or inspections, none of which have been obtained. <br /> 1 <br />
