City of Everett <br /> ' Everpark Garage Repairs Addendum No.4 <br /> 1 <br /> EVERETT <br /> WASHINGTON � <br /> ADDENDUM NO. 4 <br /> ' TO <br /> EVERPARK GARAGE REPAIRS <br /> #2018-034 <br /> ' June 28, 2019 <br /> NOTICE TO BIDDERS <br /> Addendum No. 4 contains the following revisions, additions, deletions and classifications and is <br /> ' hereby made a part of the contract documents for the above named Project and shall be <br /> considered by all prospective bidders in submitting bids. <br /> Bidders must acknowledge receipt of addendum No.4 in the space provided on page section 00 <br /> 41 13-2 in the revised Bid Form. Failure to comply with this requirement may render the Bid <br /> ' non-responsive and may cause rejection. <br /> This Addendum No.4 consist of 3 pages, including all revisions, attachments. <br /> SPECIFICATIONS, PROPOSAL AND CONTRACT DOCUMENTS <br /> ITEM 1— Per Addendum #3 answer Item 11, c., we are to assume all paint contains lead. If this <br /> ' is so, the paint will have to be removed prior to deteriorated overhead concrete removal <br /> several inches past the anticipated patch dimensions so workers will not be exposed during <br /> demo operations. Are these overhead patches and the surrounding concrete where the LBP is <br /> removed to be coated after the work is complete? <br /> Response: Painting of the ceiling is not required after the patching is complete. <br /> ITEM 2—Are we to submit our phasing plan with our bid or is this to be a submittal after <br /> award? <br /> Response: The phasing plan should be submitted after the project is awarded,to allow <br /> coordination with the City of Everett. See Addendum 3, Item 8. <br /> ' ITEM 3— Per Addendum #3 we are to apply the polymer sealer to the elevated concrete levels <br /> not currently coated with traffic membrane.
